organize, and direct compassionate care focused on comfort,
dignity, and quality of life. You will support patients and
families through skilled clinical care, expert symptom management,
and meaningful education—guided always by our values of being

Responsibilities Perform comprehensive initial and ongoing patient
assessments to determine home health needs. Develop, implement, and
update individualized plans of care in collaboration with
physicians and the care team. Provide direct skilled nursing care,
including medication administration, treatments, and preventive and
rehabilitative interventions. Identify changes in patient condition
and communicate promptly with the physician to obtain updated
orders. Educate and counsel patients and families on care needs,
disease management, and discharge planning. Serve as case manager
when assigned, coordinating care for an assigned caseload. Complete
accurate, timely documentation in the electronic medical record.
Participate in case collaboration, supervise home health aides (as
assigned), and support care plan oversight. Participate in on-call
rotation as needed. Qualifications Graduate of an accredited school
of nursing. Current RN license in state of practice and CPR
certification. One (1) year of home health experience preferred.
Strong clinical judgment, communication, and critical thinking
skills. Valid driver's license, reliable transportation, and
required insurance coverage.
